Familia Dental will be coming to the school to do a mobile dental clinic for our students on Wednesday, April 15th. They are able to provide an exam, cleaning, fluoride and sealants. They accept most insurances, if you don't have dental insurance it will be a $49 fee for an exam, cleaning and fluoride treatment. 

Attached are informational flyers with a QR code you can scan to fill out the required forms. They must be filled out entirely! If you need a paper copy you can get one from Jenny in the office. If you do a paper copy you will need to return it before April 1st, it cannot be turned in the day of the clinic.

There are limited spots available and they will see kids on a first-come, first-served basis based on when they receive the consent form, preferably online, but physical papers will be ok if needed (again, they need to be returned before April 1st.)
